One problem, with a severity and a piece of evidence a reader can chase
A finding with no evidence is an opinion. Every repository in the estate uses the word this way, and the dated findings documents are its plural.
The unit of what an investigation produces. Not a conclusion, not a recommendation: one thing that is wrong or notable, graded, and pointing at the file, command or URL that shows it. A document of findings is evidence; a document of opinions is not, however well argued.
