Definition
A confident, fluent statement by a model that is not true — a function that does not exist, a flag that was never there, a test result that was not run
The fix is not a better model; it is a harness that checks. An agent that can run the test does not have to remember the result, and a claim that carries the command that verifies it is worth more than a claim alone.
